Meet the Team
Joseph P. McDonough, MSN, MHCDS
Founder and Chief Executive Officer
Joe is an entrepreneur, innovator and leader who has redefined home health care over his 25 years of working across the care continuum and leading organizations. After graduating from Yale University with his Masters of Science in Nursing, he took his passion for population health and paired it with a commitment to focus on a significant underserved patient population in Massachusetts—those struggling with complex mental health needs and concurrent medical issues. Joe founded Innovive Health in 2000, which is now the largest home health agency in Massachusetts—and the most innovative one in the country serving this patient population. Under his leadership, the company has consistently delivered strong clinical outcomes and is now utilizing cloud-based technology tools to better understand how social determinants influence care. A pioneer in this industry, Joe is now focused on partnering with Accountable Care Organizations (ACOs) and other value-based care models to help them attain improved outcomes and cost savings for their patients in the community.
Kristen Palumbo, MHCDS, BSN, RN, NE-BC, CCE
Chief Operating Officer & Chief Compliance Officer
As COO and Chief Compliance Officer, Kristen works closely with the leadership team to implement strategies across the organization to support Innovive Health’s mission to provide innovative, compassionate home healthcare to a diverse population of complex patients with multiple chronic medical and psychiatric disease processes. She ensures that Innovive’s clinical team provides exceptional care that promotes increased independence, addresses social determinants of health and improved patient outcomes in the lowest cost care environment.
Kristen joined Innovive full-time in 2005 as a Case Manager. Since then, she has conducted over 20,000 patient visits and has held numerous leadership roles including Director of Quality Improvement, Regional Director of Clinical Services and Vice President of Clinical Services. She has served as a point person for many of the company’s valued payor, regulatory and community partners and has been at the forefront of innovative programs such as the New Graduate Nurse Residency Program and telehealth-based wound care program. Kristen is board certified as a Nurse Executive.
Kristen received her Bachelor of Science in Nursing from Salem State University. She recently graduated from the Master of Health Care Delivery Science program at Dartmouth College, and she is currently serving as Master of Health Administration (MHA) Mentor for a group of students participating in the program. In addition, she is pursuing a second Master of Science degree in Healthcare Fraud, Waste, and Abuse Investigations from the University of New Haven’s Lee College of Criminal Justice and Forensics. As a leader in nursing and healthcare education, Kristen serves as the executive sponsor responsible for Innovive’s expanding list of academic partnerships, and she has previously served as an adjunct clinical faculty member.
Anthony Loumidis
Chief Financial Officer and Treasurer
Anthony brings more than 30 years of experience to his role of CFO and Treasurer at Innovive Health. Working with senior leadership, he has streamlined the financial operations of the company by reducing expenses, implementing cost control measures and negotiating contracts with vendors. He has also played an integral role in creating a scalable financial blueprint for Innovive to increase adoption of the company’s unique model of care. Since joining Innovive in November 2017, Anthony has leveraged his knowledge of financial planning, reporting, compliance, budgeting, business valuations, investor relations, investment banking, and has worked with private equity and institutional investors. Anthony started working at Thermo Fischer Scientific Inc. and during his career, he has served as Chief Financial Officer of various companies including GeNO LLC, American DG Energy Inc., and EuroSite Power Inc. Anthony received a Bachelor of Science degree in Business Administration from the American College of Greece and an MBA from Northeastern University.
Dave Ledoux
Chief Information Officer
In his role as CIO, Dave ensures that the most innovative methods and industry-leading tools are leveraged across Innovive’s clinical and operations teams to create high-performing processes and teams. Dave was drawn to the home healthcare field due to its potential for change and the opportunity for technology infusion. Since joining Innovive Health in 2018 as the VP of Technology Services, he’s been instrumental in driving the company’s vast technology advances – shifting the company into the cloud and positioning the company for scalable growth. His early efforts to mobilize the Innovive workforce to better serve the agency’s patient population has enabled the company and its clinicians to efficiently handle the COVID-19 crisis from remote locations. With over two decades of implementing next-trend technologies and providing end user support, Dave has held roles at Okta, Inc., athenahealth, and several education and technology organizations. He received bachelor’s degrees from Rochester Institute of Technology and SUNY Oswego and he holds several licenses and certifications.
Nicholas D’Addabbo
Vice President of Strategy
As Vice President of Strategy, Nick develops and manages Innovive Health’s relationships with Accountable Care Organizations (ACOs), hospitals, physician organizations, payers and other strategic partners to build and ensure continuous alignment in care models, patient experience and revenue cycle. Since joining Innovive in 2018, Nick has worked alongside a team that has implemented platforms to help innovate and deliver better clinical care for our underserved patient population. Prior to Innovive, Nick was responsible for a large part of athenahealth’s national interoperability strategy, representing over 66,000 providers to drive clinical documentation exchange across disparate systems. Nick also has experience in the pre-hospital and acute care environments. His work across the continuum fueled his career in post-acute, ensuring that the experience for patients and families remains seamless as more healthcare shifts to being delivered in the home. He obtained his Bachelor of Science in Commerce & Business Administration, majoring in finance and healthcare management from The University of Alabama. He recently graduated from the Master of Health Care Delivery Science program at Dartmouth College.
Diane Daley, ASN, RN
Vice President of Clinical Program Implementation & Policies
As the VP of Clinical Program Implementation & Policies, Diane works with the senior leadership team to bridge the functions of the Clinical, Financial and Operations Departments to ensure a streamlined process and cost-efficient bottom-line. Diane first joined Innovive Health in 2006 as a Case Manager and worked her way up through the company to her current role. Diane has more than 20 years of experience in the healthcare field, with 15 years as a nurse, making her uniquely suited for this role as does her passion for the population that Innovive Health serves. She graduated from Northern Essex Community College with an Associate’s Degree in Nursing and holds several healthcare finance certifications.
Kimberly DiFruscia, BSN, RN, COSC-C, COQS
Vice President of Clinical Operations
Serving as Vice President of Clinical Operations, Kimberly oversees the direction, planning and execution of Innovive Health’s clinical programs to ensure patients have access to the high quality, complex care that they need to live safely in their homes and communities. Given her work as a nurse for over a decade, she understands that behind every phone call and piece of paper is a patient who needs help. Since joining Innovive Health in 2007, Kimberly has served in a number of roles including Director of Quality Improvement, Director of Per Diem Staff Services, and Case Manager. During her tenure at Innovive, she has worn many hats serving as a member of the Third-Party Liability Team, and has testified on behalf of Innovive Health during administrative law hearings. This diverse background has given Kimberly a strong understanding of home health care, the regulations that govern it, and the processes needed to succeed in this fast paced, ever-changing environment. Prior to joining the Innovive team, Kimberly was an inpatient psychiatric nurse at Tewksbury State Hospital. She received a Bachelor of Science in Nursing (BSN) degree from Southern New Hampshire University. She is also a Registered Nurse (RN), Certified Oasis Specialist-Clinical (COS-C), a Certified Oasis Quality Specialist (COQS) and certified by CITI in Social/Behavioral Research with human subjects.
Michelle Haskins
Vice President of Clinical Services
As Vice President of Clinical Services, Michelle ensures that Innvovive Health’s patients are receiving the highest-quality care from her team of dedicated nurses. She also works with a team of Clinical Directors to meet and exceed key performance indicators to coordinate care across our system. Michelle joined Innovive Health in 2013 as a Per Diem nurse in the Fall River area. Since then she has worked as a per diem on the road and full time on-call. She also served as Clinical Director in Stoughton where she built a strong, effective team of nurses who were able to successfully increase the census of the Stoughton office, while maintaining and meeting all directed key performance indicators. A nurse for more than 17 years, Michelle previously worked as a nurse manager in the Emergency Department at Saint Anne’s Hospital in Fall River. She received her Associate’s Degree in Nursing from Bristol Community College in 2004 and her Master’s of Science in Nursing Leadership Degree from Walden University in 2014.
Erenn-Leigh Kosar, SHRM-CP
Vice President of People Operations
As Vice President of People Operations, Erenn-Leigh drives all of Innovive’s people-related initiatives including employee relations, performance management, talent acquisition, benefits, communication, engagement and retention, learning and career development, and workforce planning. Since joining Innovive Health in 2016 as a Human Resources Project Manager, she has worked her way through the ranks to the VP role where she serves as strategic and tactical business partner to Innovive’s executive leadership team. Recently, she served on the company’s COVID-19 Task Force ensuring the clinical and operations staff received the integral information they needed to perform their jobs safely for the company’s vulnerable and underserved population. Erenn-Leigh has more than 15 years of experience in human resources in several sectors including healthcare and insurance. She earned a bachelor’s degree from Saint Anselm College, holds a SHRM-CP Certification and is a Crucial Conversations Certified Trainer.
Nollaig Morris, MSN, RN, FNP-c
Vice President of Quality Improvement & Education
As Innovive Health’s VP of Quality Improvement & Education, Nollaig is responsible for the overall compliance with state and federal guidelines and oversight of the Quality Improvement, Education, and Occupational Health departments. She collaborates with the leadership team and other key stakeholders to identify, develop, and execute on clinical quality improvement and education initiatives that are in alignment with regulatory requirements, internal policies, and evolving healthcare trends. Since joining Innovive Health in 2007, Nollaig has served in several critical roles, including Case Manager, Clinical Manager of TPL/ALJ, Director of Compliance, and Senior Director of Clinical Engagement, where she has contributed to the agency’s growth every step of the way. A nurse for nearly two decades, Nollaig has been steadfast in her commitment to providing quality, safe care to every single one of her patients. She served as an Emergency Department Nurse at Massachusetts General Hospital prior to joining Innovive Health. Nollaig is also a Family Nurse Practitioner (FNP-c) and has been certified as a Sexual Assault Nurse Examiner (SANE). She received her registered nursing degree from North Shore Community College and both her Bachelor’s and Master’s Degree in Nursing / Family Nurse Practitioner at Chamberlain University. Nollaig currently serves as an Adjunct Faculty – Clinical Instructor for Laboure College for Healthcare in Milton, MA.
Dean Poirier
Vice President of Finance
In his capacity as VP of Finance, Dean works with the CFO to deliver financial reporting and forecasts, perform treasury functions, and provide crucial financial and operational KPI data and analysis. Since joining Innovive in 2016, Dean has overseen the analysis and recommendations for insurance coverages, employer medical insurance plans and retirement plans. He is responsible for the generation of required cost reporting data and analysis to state agencies. Prior to joining Innovive Health, Dean was the CFO of EDCO Collaborative, a not-for-profit organization providing special education programs and services for at-risk children. Throughout his career, Dean has worked with entities that provide medical and education services for communities in need. He received his undergraduate degree in accounting from the University of Massachusetts. Dean is involved with the community and volunteers with local organizations that provide medical care for indigent patients and food for families in need.
Kacie Albritton, BSN, RN
Senior Director of Clinical Services
As a Senior Director of Clinical Services at Innovive Health, Kacie is dedicated to enhancing the quality of care provided to the agency’s behavioral health patients. Working closely with a team of Clinical Directors, Managers, and nurses, she focuses on restoring balance and independence to patients’ lives, empowering them to manage their health effectively at home and reduce the risk of hospitalizations. Kacie’s journey in nursing began 12 years ago, starting as a registered nurse on an acute care floor at a nursing home, where she gained invaluable experience in patient care. She also has a background in the urgent care setting, which further sharpened her clinical skills. Kacie joined Innovive Health in 2013 as a Per Diem nurse, quickly advancing to roles as Clinical Manager and Clinical Director in the Fall River area, where she played a crucial role in developing and implementing effective care strategies. Kacie holds a Certificate in Nursing from Brockton Hospital and a Bachelor of Science in Nursing (BSN) from Chamberlain School of Nursing. Her commitment to delivering compassionate, high-quality care is evident in her leadership and the positive impact she has on both her team and the patients they serve.
Rose Gakuru
Senior Director of Clinical Services
In her role as Senior Director of Clinical Services for Innovive Health, Rose provides leadership and encouragement to her direct care clinical teams to ensure they are well equipped to perform their duties. Rose has been a nurse for nearly 15 years and was drawn to join the Innovive team in 2012 because of her passion for patient care and teamwork. She started out as a Per-Diem nurse and moved up through the ranks to Case Manager and Clinical Manager before assuming her current position. Rose said one of the highlights in her role is being able to successfully coach her nurses and watch them turn into leaders along the way. Prior to joining Innovive, Rose worked at Wedgemere Nursing Home as a floor nurse and was a Licensed Practical Nurse (LPN) from 2006-2013 before receiving her Registered Nurse (RN) license. She graduated with an Associate’s Degree in Nursing (ADN) from Massasoit Community College.
Daniel Politano
Senior Director of Clinical Services
As a Senior Director of Clinical Services, Dan provides leadership and support to Innovive Health’s direct care clinical teams of clinicians focused on providing skilled nursing services to patients, who have multiple medical comorbidities with complicating behavioral illness. Having overseen several of Innovive Health’s regional offices in Massachusetts and Colorado, Dan has a proven track record of building and leading collaborative clinical teams. When he first joined the agency in 2012, Dan served as a case manager, which provided him with first-hand experience providing care to complex behavioral health patients in the home setting along with invaluable knowledge of the home healthcare industry. Prior to Innovive Health, he worked as a registered nurse for Taunton State Hospital, and he has clinical experience working for various medical centers including Saint Elizabeth’s Medical Center and Caritas Carney Hospital. He received his Associate of Science in Nursing from Caritas Laboure College, and a BSN from Southern New Hampshire University.
Stephen Porcella
Senior Director of Clinical Services
As a Senior Director of Clinical Services, Stephen provides leadership and support to Innovive’s direct care clinical teams and actively engages with community health partners to promote collaborative care for patients. Stephen joined Innovive Health in 2007 as a Per-Diem nurse and then was promoted to the roles of Case Manager and Clinical Manager before assuming his current role. In this role, Stephen has led his team through the successful implementation of Innovive Health’s Electronic Medical Record platform. He was also key to assisting the organization navigate the COVID-19 crisis—ensuring patients received uninterrupted high quality care—even in the face of unprecedented obstacles. A nurse for over two decades, Stephen previously worked with disabled veterans as Nurse Clinical Manager for Massachusetts Executive Office of Health and Human Services. He received his Bachelor in Business Management from the University of Massachusetts and his Associates Degree in Nursing (ADN) from North Shore Community College.
