In 2004, Innovive Health was founded in Massachusetts with the mission of servicing the population’s most vulnerable and underserved citizens at home to ensure that they were able to remain successful and independent within their communities.
Our care model was designed to focus on patients who have multiple medical comorbidities with complicating behavioral illness, treating and educating patients on their holistic well-being. In addition, our model concentrates not only on a patient’s mental and physical illness, but also the social components that impact clinical outcomes.
Since our inception, we have continued to play a vital role as the hub for a complex care team – coordinating with a multitude of specialists and providers across the continuum of care. We have built an innovative care model that enables us to effectively work with this population to manage and educate them on their disease, lifestyle, and coping mechanisms. In the process, we have established and expanded our partnerships with healthcare organizations and world-class technology companies to fundamentally change the way care is delivered to this population from referral through graduation of a patient to full independence.
To reflect our evolution and our best-in-class model, we rebranded the company as Innovive Health in May of 2021. Our new brand reflects our commitment to re-engineering how home health care is delivered to complex and vulnerable patients across the nation, leveraging technology and data as a platform for enhanced clinical decision making along with the elimination of social barriers to care and the reduction in the utilization of high-cost care environments.
At Innovive Health, we fundamentally believe in being a resource to our patient and the entire continuum of care. We are excited about the future, and we will continue to be a center for innovation in home care by investing in the ongoing development of our clinical team and operational employees to provide better outcomes, more successful patients & communities, and a greater emphasis on accountable care.
